The liver is located on the right side of the abdomen, just beneath the rib cage, so pain is often felt in this area first. The most common symptom is a dull, aching pain or a sensation of fullness or pressure in the upper right quadrant of the abdomen.

As the liver enlarges, it may also stretch the capsule surrounding it (the Glisson’s capsule), which contains nerve endings that can trigger pain. This pain may radiate to other areas, such as the back, specifically the right side of the lower back or between the shoulder blades, as the swelling presses on surrounding tissues and nerves.

Swollen liver can also cause discomfort in the abdomen, often leading to bloating, indigestion, and a feeling of heaviness. If the disease progresses, other symptoms can arise, such as jaundice (yellowing of the skin and eyes), swelling in the legs and ankles, and dark urine, which may cause discomfort throughout the body. Additionally, liver diseases can lead to referred pain, where issues with the liver cause pain in distant areas like the shoulders, especially the right one.
